Wellness and Reablement Lead
At Brotherhood of St. Laurence (BSL), we are seeking a passionate Wellness and Reablement Lead to join our dedicated team Aged care team.
Location: Frankston
Job Type: Max term 12 months working 38 hours per week
Salary: $87,886.87- $94,565.36 per annum, based on skills and experience plus 12% superannuation. (pro-rata for part-time employment)

About the role
The Wellness and Reablement Lead partners with participants by using these approaches to focus on individual participant strengths and goals, with the aim of supporting the improvement of the participant’s wellbeing.
Key Responsibilities
- Service Delivery
- Teamwork and Accountability
- Ensure services focus on supporting participants to achieve their agreed goals as outlined in the participant’s care plan
- You’ll play a vital role in ensuring services are tailored to meet individual client needs, goals, and preferences
- Appropriately manage performance and coaching support to the Community Support Workers, providing regular informal and formal performance feedback and reviews, supervision and support for relevant training and development.

What we are looking for
We are seeking an enthusiastic and motivated individual who has a passion for supporting the mission of BSL and delivering excellence in your role.
To be successful in this role, we are looking for someone who has the following key skills and experiences:
- Diploma-level qualification in Community Services, Aged Care, or Disability Care
- Demonstrated experience in effectively leading and/or managing teams, including providing coaching, performance feedback, and professional development opportunities to support individual growth and enhance team capability.
- Experience in co-designing care plans according to participants goals, preferences, needs and requests, and in line with relevant aged care funding guidelines, legislative requirements and organisational policies and procedures.
- Experience in navigating conflict, and diffusing challenging situations through the use of active listening, open communication, body language and intonation.
